2014 International ProStar - Heavy Steering and Shuddering

My 2014 ProStar has developed a very heavy steering feel, specifically when I'm backing into a spot or turning at low speeds. It feels like I have no power steering at all for a split second, then it kicks back in. When I turn the wheel while stopped, I get a shuddering through the steering column. Fluid level is full and doesn't smell burnt. I don't see any leaks around the steering gear box or the pump. The belt is tight and not slipping. Could this be the pump failing internally or is the steering box shot? Truck has 600k miles.
