Question

Running down I-80 and the dash lit up like a Christmas tree. I've got a 'SCR System Fault' and the truck dropped me into a 5 MPH derate 50 miles later. The DEF gauge is showing empty even though I just topped off at the Pilot. I'm sitting on the shoulder and DAVIE is showing communication errors with the aftertreatment system.

I checked the fuses in the engine bay and everything looks solid. I tried a manual regen but it won't even kick off because of the active derate. I'm worried the SCR controller or the DEF header itself has kicked the bucket, but I don't want to spend $1,200 on a part if it's just a wiring issue.

✅ Confirmed fix: The issue ended up being the DEF quality sensor/header unit itself. This is a known failure point on the 2018-2019 models due to internal coolant leaks onto the circuit board inside the header.

I replaced the DEF header assembly (Part# 2125573PE) and cleared the codes with DAVIE. The derate cleared after a short test drive to let the ECM see the new sensor readings. If you're stuck, you can sometimes bypass the coolant flow to the tank to prevent it from getting worse until you get the part.

Have you checked the 4-pin connector at the DEF tank header? These Paccars are notorious for green crusties getting into the CAN bus wires right at the tank. If the level sensor can't talk to the ECM, it defaults to empty and triggers the derate immediately. Clean it with some contact cleaner and check for 60 ohms across the yellow/green wires.
