2016 Ram 2500 - P2002 Particulate Trap Efficiency Below Threshold - DPF Help

I'm fighting a P2002 on my 2016 Ram. The truck keeps going into active regen way too often, sometimes every 30-50 miles. I've noticed the exhaust tips are starting to get some black soot on them, which I know shouldn't happen with a healthy DPF. I've checked for intake leaks and replaced the air filter, but the code keeps coming back after I clear it and drive for a bit. My soot load stays high even after a long highway drive. I’m worried the DPF is actually cracked internally or the pressure sensors are lying to the ECM.

Already tried
